Microplastic exposure is an emerging issue of concern for infants exposed to multiple sources of these tiny particles every day. Microplastics are small plastic fragments that are less than 5 millimeters in size. Microplastic routes of exposure include inhalation, dermal contact, and ingestion. Microplastics have been found in air (synthetic textiles, degradation of plastics, building materials, waste incineration, and landfills), face masks made of synthetic fibers, soil, indoor dust, tap and bottled water, beverages, honey, salt, sugar, tea bags steeped in hot water, seafood, plastic food packaging, dairy milk, soap, cosmetics, hair bleaches, hair colorants, body lotions, lip care products, deodorants, and nail care products. Of concern to lactation personnel and childbearing families is that microplastics have been found in both breastmilk and infant formula. Breastfed infants are exposed to microplastics not only through ingesting milk directly from the breast but also if expressed milk has been stored in plastic breastmilk storage bags.Higher levels of microplastics may negatively affect lactation and have been associated with maternal mastitis, breast engorgement, low milk production and have been shown to alter the bacterial microbiome of breastmilk. Microplastic exposure in cells and laboratory animals can cause cell death, inflammation, and metabolic disorders. Since human milk is synthesized at the cellular level, heavy microplastic loads could have the potential for disrupting the workings of the lactocytes (milk secreting cells). Clinical implications will be presented and ways to lower microplastic exposure will be discussed.
